Initial outline

Just started with a general outline, and began describing the major symptoms and signs. Much of the pathology, clinical stuff and treatment would fall under neuropathy as such, or some disease which causes neuropathy, so that the description of the severity and intractability of the pain, and the attendant loss of quality of life, is the main thrust of this outline.
